Mental Health Support and Services
At Pepin County Human Services, we are committed to supporting the mental health and well-being of our community members. Our programs provide comprehensive, person-centered services to individuals of all ages facing mental health challenges, substance use concerns, or disabilities. Below, explore our key programs designed to promote recovery, independence, and quality of life.
Comprehensive Community Services (CCS)
Comprehensive Community Services (CCS) is a voluntary, Medicaid-funded program that offers psychosocial rehabilitation for children, youth, and adults with mental health or substance use challenges. CCS is client-driven, focusing on individualized recovery plans to help participants achieve their personal goals.
- Eligibility: Pepin County residents who are Medicaid-eligible and meet functional requirements through a screening process.
- Services: Includes care coordination, therapy, skill development, peer support, and assistance with daily living needs.
- Recovery Team: Each participant works with a dedicated team, including a service facilitator, mental health professionals, and natural supports like family or friends, to create and implement a recovery plan.
- Goal: To empower individuals to manage their health, enhance wellness, and live independently in their community.

Community Support Program (CSP)
The Community Support Program (CSP) provides coordinated care for adults living with severe and persistent mental illness. CSP offers intensive, community-based services to reduce symptoms, promote stability, and support recovery.
- Eligibility: Adults with a severe mental health diagnosis that significantly impacts daily living and poses a risk of hospitalization.
- Services: Includes case management, medication management, counseling, vocational support, and assistance with daily activities.
- Approach: Emphasizes collaboration, hope, personal choice, and respect, with services tailored to each individual’s unique needs.
- Goal: To help participants achieve stability and live productive lives within the community.

Coordinated Services Team (CST)
The Coordinated Services Team (CST) is a family-centered, community-based initiative designed to support youth with significant behavioral health needs. CST brings together families, service providers, and community resources to create a coordinated care plan.
- Eligibility: Youth with behavioral health challenges requiring coordinated support across multiple systems (e.g., school, healthcare, social services).
- Services: Includes service coordination, family support, crisis planning, and access to mental health or substance use treatment.
- Approach: Empowers families to lead the planning process, ensuring services align with the youth’s and family’s goals.
- Goal: To improve outcomes for youth by fostering collaboration and providing wraparound support.

Additional Support Services
In partnership with the Western Region Recovery and Wellness Consortium (WRRWC), which includes Pepin, Buffalo, Chippewa and other area Counties, we offer additional mental health and disability services, such as:
- Crisis Services: 24/7 telephone crisis support through Northwest Connections, with mobile crisis response available from 4 p.m. to midnight daily.
- Outpatient Services: Counseling and therapy for individuals seeking less intensive support.
- Contracted Services: Access to specialized mental health and substance use treatment providers.
These services aim to maximize access to care, improve quality, and promote recovery-oriented, community-based support.
