Monday through Thursday 8 a.m. to 5:00 p.m. and Friday 8:00 a.m. -11:30 a.m. with scheduled on call time

Position Objectives:

Position works with children with special needs, to include mental health and substance abuse. This is the entry level community behavioral health services position built on recovery concepts.  Under supervision, the employee assists consumers with environmental and supportive services designed to address and overcome physical, mental, emotional health or family issues; conducts individual or family assessment to determine service needs; provides agency services when appropriate; and refers consumers for other community-based services when needed.  Services to consumers may take the form of protective actions on their behalf or on behalf of the community. 

Duties and Responsibilities:

Primary Assigned Program Area:

Children’s Long-Term Support Program and Mental Health and Substance Abuse services for adults and children.

On-call duties for the department for social services emergency issues (mobile crisis and all other department crisis services) after hours and weekends.

Office ACCESS worker on a rotation basis for all social service needs of consumers.

 

Duties and Responsibilities:

Position fulfills requirements of Service Facilitator as outlined in Wisconsin Statutes 46; 48; 49; 938; 51; 54; 55; Federal Requirements; and Certified Programs (34 and 36).

  • Performs ACCESS and On-Call duties and facilitates referrals for emergency services including child protection, juvenile justice, emergency placements of adults and elderly including mobile crisis, etc.
  • Provides community services per Wisconsin statutes, court policies, and/or requirements of program, state, or federal guidelines.
  • Provides services in support of recovery concepts and practices. 
  • Accepts requests and referrals for agency services.
  • In collaboration with the consumer and his/her supports, conducts comprehensive, strength-based assessments through interviews, home visits and collecting of collateral information.
  • In collaboration with the consumer and his/her supports, determines the range and type of services needed for preventative or rehabilitative services.
  • Explains the scope of services and discusses consumer’s rights and responsibilities in relation to the use of services.
  • Arranges or provides for appropriate services for consumers based on their assessed needs. 
  • In collaboration with the consumer, his/her supports, and other providers, assess ongoing consumer needs while developing and updating service plans to address needs. 
  • Maintains documentation that meets professional practice, billing expectations, and program standards.
  • Prepares correspondence, reports, and records as necessary and appropriate.
  • Coordinates and participates in recovery teams with the goal of bringing people together to collaboratively work with and for consumers.
  • Coordinates and monitors ongoing delivery of services to consumers. 
  • Participates in committees and task forces as assigned. 
  • Testifies in court as necessary or required.
  • Adheres to approved social worker principles, methods and practices, including the National Association of Social Workers (NASW) code of ethics.

  • Under direction of Supervisor this position may at times be required to work overtime hours or shifts.

Minimum Requirements:

  1. Bachelor’s Degree in Social Work or relevant field
  2. Good written and oral communication skills; tact and courtesy; thoroughness and dependability
  3. Knowledge of the functions, procedures, organization, policies, and regulations governing the administration of the Children’s Long-term Support Waiver and Comprehensive Community Services Program
  4. Advanced facilitation skills 
  5. Ability to maintain confidentiality in dealing with all pertinent information
  6. Ability to establish and maintain effective working relationships with peers and supervisors
  7. Valid motor vehicle operator’s license and/or available means of transportation required.
  8. Available to be on call as assigned
  9. Meets requirement of background check including caregiver

Desirable Training and Experience:

  1. Three years’ experience with families with mental health, substance abuse, and/or developmental, physical, cognitive disabilities
